What to Expect in Your First Therapy Session: A Guide for New Jersey and New York Clients
By Chanill Medina, LMFT
If you have never been to therapy before, the first session can feel like a big unknown. Most of the people I work with come into our first meeting with the same handful of questions. What will I have to talk about? How personal is too personal? Am I doing this right? This guide walks you through what to actually expect in a first session at Generational Wellness Center, so you can show up with a little less worry and a little more clarity.
Before the Session
Every client begins with a free 15-minute consultation. This is a short video call where we get a feel for whether we are a good fit. You can ask me anything you want about my approach, my training, or how I work with specific concerns like anxiety, family dynamics, or identity. There is no commitment after this call, and nothing you share goes into a clinical record yet.
Once we decide to move forward, I will send you intake paperwork through SimplePractice, my secure records system. It includes a short personal history, your goals for therapy, and consent forms. You complete it on your own time before our first full session.
The First 60 Minutes
Our first session is 60 minutes, just like every session that follows. I run all sessions virtually through a HIPAA-compliant video platform. You can join from your home, your car, or anywhere you have privacy and a stable connection.
The first session is mostly about getting to know you. I will ask about what brought you in, what you have already tried, what your day-to-day life looks like, and what you would like to be different a few months from now. You do not need to have all of that figured out. Many people walk in saying “I just know something has to change.” That is enough to start.
Things you might be asked
- What is happening in your life right now that made you reach out
- How long you have been carrying what you are carrying
- Who knows about it, and who does not
- What your family of origin was like growing up
- What support, if any, you have outside of therapy
- What you hope therapy will help you do or feel differently
You can answer as much or as little as you are ready to. There is no quota. You are not on trial. We move at your pace.
What I Am Doing While You Talk
While you share, I am listening for patterns. I am noticing what you say a lot, what you avoid, what makes you light up, what makes you tense. I am thinking about how your story connects to family systems, trauma responses, and the modalities I work with such as Cognitive Behavioral Therapy, Solution-Focused Therapy, and the Gottman Method for couples. None of that comes out as jargon in our session. It is the structure underneath the conversation, not the conversation itself.
What You Walk Away With
By the end of the first session, you will usually leave with three things. A clearer sense of what you actually want to work on. A small, concrete next step to take in the week ahead. And a sense of whether this feels like a space you want to keep showing up for.
If at any point in our work together it does not feel like the right fit, you can say so. I will help you find someone who is a better match. The goal is your growth, not my caseload.
A Few Practical Notes for NJ and NY Residents
- I am licensed in New Jersey and New York. You can see me from anywhere within either state.
- Individual sessions can be billed to United Healthcare, Aetna, or Horizon. Couples and family sessions are private pay.
- Cancellations require 24 hours notice to avoid the full session fee.
- You do not need a referral or a diagnosis to start.
If You Are Still Unsure
Most people who hesitate to start therapy do not hesitate because they do not need it. They hesitate because they are not sure what they are getting into. A free consultation is the lowest-stakes way to find out. Fifteen minutes, no record, no fee, no pressure.
If you are in New Jersey or New York and you want to see what it could feel like to have someone in your corner, I would love to meet you.
